Definition
As a member of the social services team, provide direct task-oriented social work services to clients/patients, families, and others; and/or participate in the practicum instruction of social work assistant students.

Under general supervision, provide direct task-oriented social work services and/or participate in the practicum instruction of social work assistant students.
Typical Work
Assists clients/patients, families, and others with discharge planning; coordinates with other staff members; explains program requirements (such as Medicaid, Medicare, Social Security Disability) to potential participants and referrers;
Participates in practicum instruction of social work assistant students;
Teaches clients/patients, families, and staff task-focused aspects of discharge planning;
Performs the work of Social Work Assistant 1's;
Performs related duties as required.

Desirable Qualifications
Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences
AND
Two years experience as a Social Work Assistant 1 or equivalent.
OR
Associate of Arts Degree in the behavioral sciences
AND
Four years of work experience as a Social Work Assistant 1 or equivalent.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
