Definition

Performs professional pharmaceutical care to a clinical service such as cardiology, neurosurgery, critical care, transplant services, oncology, psychiatry, diabetes or family or veterinary medicine; provides leadership for specific clinical service areas such as developing education programs and acting as clinical resource for staff.

Distinguishing Characteristics

Performs specific leadership functions to a clinical service related to clinical practice, education and standards of pharmaceutical care such as recommending appropriate pharmaceutical prescription, influencing standards of therapy, and developing the pharmacy component of a clinical service.

Typical Work

Recommends appropriate pharmaceutical prescription based on patient parameters and interpretation of current literature, safety, efficacy and cost; 

Advises prescribers on patients’ medication therapy; 

Assists in the Pharmacy and Therapeutics Committee process for review and evaluation of treatment options; 

Maintains accurate medication histories and monitors effect and adverse reactions of all patients; 

Participates in multi-disciplinary team activities to provide patient-specific medication information and education when appropriate; 

Evaluates information from prescriber or interviews patients and provides education or assistance for the self-administration of take-home or discharge medications;  

Assists in problem resolution with the healthcare team; 

Directs the work of and instructs pharmacy students, residents, interns and/or pharmacy assistants; 

Assists in the development, implementation, and evaluation of clinical pharmacy services such as clinical pharmacy, specialty residencies, workshops, and continuing education programs in the clinical service; 

Participates in development, implementation and evaluation of departmental policies; 

Participates in the development and documentation of quality assurance indicators such as drug use evaluation; 

Develops and presents periodic in-services to healthcare providers; 

Participates in pharmacy and hospital committees; 

Participates in research studies in the clinical service; 

Performs the duties of a Pharmacist 2;
Performs related duties as required.

Knowledge and Abilities

Knowledge of: pharmacology, pathophysiology, pharmacokinetics, current medical literature and techniques of critical evaluation, clinical drug usage, patient monitoring techniques, drug blood level analysis and interpretation.   

Ability to: critically evaluate drug related medical literature; effectively communicate with other professional and paraprofessional staff, both verbally and in writing, concerning appropriate drug utilization; act as an effective liaison between medical, nursing, and pharmacy staff; provide accurate drug information to physician and nurse when required; evaluate patient drug therapy; monitor drug therapy progress; prepare and deliver in-service seminars to medical, nursing, and pharmacy staff.

Legal Requirement(s)

State of Washington Pharmacist License.

Persons legally authorized to work in the U.S. under federal law, including Deferred Action for Childhood Arrivals recipients, are eligible for employment unless prohibited by other state or federal law.

Desirable Qualifications

Two years of experience in pharmacy practice.  Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.
